We provide a variety of continuing care services to meet individual goals for recovery, health and wellness. The individuals we serve may be in need of long-term care skilled nursing, shorter-term comprehensive inpatient nursing and rehabilitation, or outpatient rehabilitation services. Our Center’s philosophy of care combines a holistic approach to serving individuals. We are proud to be one of the newest and largest facilities in the region. Our 300-bed facility is approved to participate in Medicare and Medicaid programs and maintains participating agreements with a variety of Medicare Advantage and other managed care plans. Our interdisciplinary staff strives to meet individual goals toward recovery and overall health while offering an environment that is enriching in activities and socialization. The Center has the services of a full-time Medical Director and a dedicated team of professional personnel and geriatric specialists from regional academic health care centers. Admission to Skilled Nursing and Long Term Care Admission to the skilled nursing and long-term care programs is based on a medical evaluation, most often completed in a hospital.
