Our Center is an enriching therapeutic environment with physical
amenities second to none in the region. We provide a variety of
continuing care services to meet individual goals for recovery, health
and wellness. The individuals we serve may be in need of
long-term care skilled nursing, shorter-term comprehensive inpatient
nursing and rehabilitation, or outpatient rehabilitation services.
Our Center’s philosophy
of care combines a holistic approach to serving individuals. We are
proud to be one of the newest and largest facilities in the region. Our
300-bed facility is approved to participate in Medicare and Medicaid
programs and maintains participating agreements with a variety of
Medicare Advantage and other managed care plans. Our interdisciplinary
staff strives to meet individual goals toward recovery and overall
health while offering an environment that is enriching in activities and
socialization. The Center has the services of a full-time Medical
Director and a dedicated team of professional personnel and geriatric
specialists from regional academic health care centers.
Admission to Skilled Nursing and Long Term Care
Admission to the skilled
nursing and long-term care programs is based on a medical evaluation,
most often completed in a hospital. To ensure a smooth transition from
acute care to long-term care, nursing staff at the Center work closely
with their counter-parts in area hospitals to ensure a smooth transition
of a continuing care. Our commitment to provide individualized care

Comprehensive
Inpatient Rehabilitation Services
For individuals in need
of short-term rehabilitation services, our inpatient program provides
comprehensive coordinated services for individuals following a
hospitalization. Often, this service provides the individual the
care required to maximize their level of independence to return home or
to transition safely to long-term care living arrangement.
We are proud to offer state-of-the-art rehabilitation services by a
dedicated interdisciplinary team who together carry out a treatment plan
designed to meet each person’s needs, goals and preferences.
Services are provided in a therapeutic environment with the latest in
rehabilitation equipment and technologies.
An individual’s treatment team may include the following:

- Occupational Therapist
- Speech Pathologist

- Recreation and Activities
The Center is licensed to provide rehabilitation services to
individuals on an outpatient basis. Individuals recovering from a
recent illness, surgery or hospitalization may benefit from outpatient
services. These services include physical therapy, occupational
therapy and speech/language pathology. The outpatient
rehabilitation program serves individuals with a wide range of needs and
conditions. We provide care to those with orthopedic conditions
such as fractures, joint replacements or back pain, in addition to those
with neurological problems.
